I finished the anime a few days ago, does the game follow the same story? Is it worth getting into?

But it is basically the same story, yeah? Well then, I guess I'll give it a go then, I really wouldn't mind going through it again, the anime was awesome.

One thing though, does it focus on telling the story or is it one of those VNs that make you jump through hoops to get the ending you want?

Its pretty straightforward. 3 of the endings are essentially "do you want to get this ending" options. They're more subtle than that, but it is pretty much a single point of divergence.
The other 3 require you to go all the way to the end and you get a different one depending on how many "triggers" you have hit, which are basically just questions through the game that need answered properly.

True End is a pain in the ass to get blind, but other than that the other endings are pretty straight forward, although the amount of people who couldn't figure out how phone triggers work has made think otherwise.
