Most satellites don't orbit this high. This was taken by Apollo 17 on the way to the moon.

No the image shows that it was taken on a sound stage. There are maps you can view to see the distance. The far right mountain is meant to be several km's away. We can see a strong parallax effect when the photo is taken a meter to the left of the other. This shows that the mountain is much closer than the maps show.

How so? The color composites look a bit odd since the camera is designed to take photos of the different color channels one at a time. Since the Earth is rotation and the Moon is moving in these photos, the different colors don't quite line up.

No, with a camera placed on the table you get the same 90deg angle with the ground, you will just need a larger table because then the horizon will be farther away from you. For the sake of simplicity and also because in the wilderness the horizon line is always at eyes' level (because earth is flat) that experiment puts the camera on the level of the table's edge.
