Those missiles are kinetic and there's a good chance the nukes won't go off.
But even if they do, in high altitude, the radiation will just radiate and it won't be THAT bad. The problem is when the radiation gets absorbed by material on the ground. The closer the explosion is to the ground, the more radioactive it will become. And a lot of it will get propelled into the air that's what causes nuclear fallout and black rain. It makes the radiation stick around. There would be much less of it if it explodes in high altitude.

The point is this type of system can only have partial success, which wouldn't matter in a nuclear war. The only benefit it gives is the Russian have to assign more missiles to major targets to assure destruction, hence sparing secondary targets.

A massed MIRV attack amounts to dozens to hundreds of warheads raining down a target.

That being said, the Topol-M carries a single strategic warhead, but can be fitted with a MIRV unit with tactical warheads.

Intermediate range ballistic missiles aren't ICBMs. Theres no defence against ICBM reentry in the terminal phase. That's why US is putting missile defenses in poland/romania etc. Because shooting down ICBMs in the launch stages is possible.

thats not how nukes work. they're extremely complex and undergo a very specific multi stage detonation (happening in millionths of a second). if even the slighest of things goes wrong the nuke will not detonate, or just 'fizzle'. for example any outside explosion would simply tear the nuke apart, since only an extremely precise spherical implosion of the plutonium core can get it to reach supercriticality.

nukes in fact have built in conventional explosives that explode in the wrong sequence, thus preventing spherical implosion which in turn means the plutonium never reaches a supercritical state and thus no nuclear chain reaction ever occurs.

Technically there is an (ineffective) counter to re-entry terminal - a system similar to a CIWS - actually basically a 30mm Goalkeeper CIWS with some adapted naval missile killing missile as a secondary.
And it required much more computing horsepower and radar array than a normal system.
It can actually engage and take out the nuke, the problem is the nuke still detonates mere feet above it's designated detonation altitude (nukes are airbursters, ground dets are less effective) but even then the failure rate (miss rate) was incredibly high

Decoys would be something like chaff that confuses the lock-on systems missiles use. The evasion just means it can steer out of the way of anything that's trying to shoot it down.
