gintama is genuinely funny from time to time, but why it is so highly regarded in the west? it's crammed full of japanese pop culture references that mostly fly over people's heads or have simply aged like milk - not to mention it has a lot of low-brow slapstick/potty humor.

when it's serious it can also be good, but tonally it's usually all over the place, and has way too much filler. it's not that it's bad, but it's not really great, either.

It's not THAT highly regarded, don't confuse it's small hardcore MAL fanbase inflating it's ratings with it's actual level of popularity

99% of the references are just other Jump manga, they're easy for anime fans to get

Episodic comedy isn't filler just because it's not part of an overarching story. The real trash is the plot driven arcs, I have no explanation for why anyone likes those.
