Nope, prolonged radiation exposure that alters DNA or "mutates" them. Now you can die from sudden concentrated radiation yeah, but ghouls come from slowly ingesting/absorbing radiation over a period of time that alters cells instead of killing. Dosage is key.

This is actually not true at all. They used radiation as a novelty for a period of time, even going so far as using x-rays to determine your shoe size. For quite a while we were fucking with radiation and didn't know it was harmful.

By Nicole Carpenter Despite the shooting in Fallout 4 being "really good," some players thought Bethesda could have improved on a few things, including the dialogue system.

Bethesda Game Studios director Todd Howard spoke to GameSpot about the studio's tendency to "try new things" and how the team's efforts don't always pan out. "Obviously, the way we did some dialogue stuff, that didn't work as well," he said.

Meanwhile Pete Hines loved the dialogue system because his attention span is too short for dialogue in other games - including Fallout 3 and New Vegas.

I don't even like Todd, but shit, maybe without Hines interfering we could have at least gotten a game as good as 3, even if it would still be nowhere close to 1/2/NV and Tactics.

They don't regenerate any faster than normal ghouls, I think, just that there's way more radiation to regen off of. And I don't think they're traditionally feral, but traditionally insane from the constant pain and isolation
