In Japan, as well as most of Asia and I think East Europe, children learn to play the Recorder. The rather simple idea is to contrast the act of playing a recorder with oral sex to try to "surprise" the viewer for arousal.

This works based on two assumptions:
1. That children are stupid and can't do anything unless an adult first explains it. Thus they couldn't possibly understand that sucking the recorder should be seen as obscene.

The first point is a belief that is by this point literally obsolete. Secondly, the information that used to only filter to children through older siblings is now available to everybody with just a couple of words typed into google.
The second point is less untrue but only on a superficial level. The current morality regarding sexual conduct is nowhere near the old fear-mongering religious one. There is still an "etiquette" surrounding it, aka you aren't suppose to talk out loud about your sex life, but there are hardly many people left that think sexual pleasure is a "sin" or that see enjoying sex as degenerate behavior.

So that's why you shouldn't feel uncomfortable, it's nothing to be affected by.
