It's a prototype Voodoo5 6000, one of the rarest video cards in the world. It was basically an SLI of four VSA 100 chips. 3dfx went under before they released it. It would have been a flop either way, since games just couldn't use all four graphics chips easily. It was barely faster than a Voodoo5 5500.

I spent about $300 total for it in late 2017. $30 for the CPU, $50 for the motherboard, $10 for the RAM. The Voodoo4 and Voodoo5 prices are all over the place now, but I got the Voodoo4 4500 for $150.
