The whole series is in the mind of a 30-something year old Malcolm, whose life is a wreck. He's jobless, depressed, and overall angry at where his life has gone. The only solace he gets is remembering his terrible childhood, and blaming it as the reason why he's so miserable, despite his other siblings having much better and happier lives than him. Unfortunately, Malcolm often gets so caught up in his daydreaming that he neglects his wife. She gets fed up with this more and more, until one day she leaves him for another man. This drives Malcolm further into madness, and he has thoughts about suicide every day as he drinks the pain away.
The door slam is Malcom snapping out of his daydreaming, because his wife slammed the door when she left Malcolm for good. In the end, Malcolm succumbs to his suicidal thoughts, and fashions a noose for himself out of a necktie Hal gave him when he moved out of the house. Then, alone in his apartment, Malcolm ends his life. It's not until 3 days later that the landlord finds his hanging body, not because he fears for Malcolm's safety, but because the rent is late again.
The image of Malcolm facepalming at the end of the intro is Malcolm burying his face into his hands in despair after his wife leaves him alone, depressed, and suicidal. "Yes, no, maybe, I don't know, can you repeat the question?" is a sly reference to the fact that Malcolm often spends his time remembering his childhood, and when he's snapped out of it (Usually by his boss or wife), he forgets where he is and what he's doing. "You're not the boss of me now" is Malcolm's hatred against the world, believing that suicide is the final act of rebellion against his terrible life. "And you're not so big" is Malcolm speaking to himself, because he realizes that his life is insignificant in the grand scheme of things, and that he's a nobody despite him being a genius. Finally, "Life is unfair" are Malcolm's final thoughts before finally losing consciousness forever.
