Also, at the end of The Doctor Falls, 12's body repeatedly glows and it seems a huge effort to fight off the regeneration energy. Yet in TUAT, he just strolls around barely bothered by it - there isn't even a slight hint that his body is on the verge of change.

Are we meant to infer, in TUAT, that by then he's refused to regenerate and is now slowly dying (see: S3 finale)? And then at the very end, he basically flips the switch back in his body and chooses to let the change happen after all? What's the deal here

Yes. Honestly I think the big change of mind moment was underplayed and was basically when he was arguing to 1 why 1 had to regenerate and realised how it could apply to him too. That just cooked over in his mind the rest of the episode.
For the record I have a lot of problems with TUAT and think it undercut a lot of the magnificence of what TDF was doing with this kind of stuff.

I suppose 10 began regenerating then spent a long while just meandering about seeing old friends, before eventually staggering to his TARDIS to regenerate. But at that point, he just collapses and regenerates after the stumble. PCap played it so that 12 went immediately into stumbling mode, but /then/ held it off for a whole episode.
