Because trans people aren't really delusional. I don't know any trans women who ever had illusions about actually being female, like thinking they were the same as a cis women. Every trans person acknowledges that there is something wrong with them, if there wasn't then they wouldn't seek treatment.
Gender Dysphoria makes you want to be the opposite sex, to look like and to be seen as it. When they see their male features in their reflections and everyone sees and treats them like a man it hurts them deeply, and as it goes on they stop wanting to continue with life and become severely depressed until they kill themselves. The only way to treat this is for them to transition so that they are happy with their body, people have wanted to cure this for thousands of years but there is no way we have discovered besides transition. Because they can continue life normally after they transition without always falling into depression or wanting to die, it makes sense to just treat them this way and let them have somewhat normal lives.

Kids can't get on hormones, once they get to around 10 they'll usually put them on puberty blockers until they're around older when they'll be allowed to decide if they want hormones.
And literally no one who was legally allowed to would give sex reassignment surgery to a kid.
