What all would this war encompass, both geographically and ideologically. Is it going to be people breaking and fighting for their ideals instead of fighting for their respective countries? Would the war be fought on US soil this time? Or do we continue forward in the Middle East or back in Europe?

Whoever America is fighting knocks out our power grid and other utilities immediately. A nuclear exchange between anyone would trigger a reactionary nuclear war. Very few would survive, if any. Radiation would drift through wind currents, poison land and water, kill wildlife.

Modern nuclear detonations are a fraction of the leaks at Chernobyl and Fukushima. Fallout is still a thing, but it's nowhere what it was. Hiroshima and Nagasaki were far dirtier than anything that might occur in the modern era.

A fuckton of people would die and it would be a miserable world for those in the hit zones, but after the nuclear winter it's smooth sailing. 19th century at best and even then people would still retain 21st century knowledge, many gadgets and tools. Rebuilding takes time and it'll be anywhere from 20 to 50 years, not 10 000 years.

You can visit the testing fields of Southwest today without any harm and they were nuked weekly for DECADES. You still can't enter much of the Chernobyl containment zone without protective equipment.

Invading the U.S. would be extremely difficult.. Any fighting done in the U.S. will likely be done among the citizens, like we already see happening. I can see WWIII not only being a war between world superpowers, but also igniting a worldwide race war and Holy war at the same time.

Guess I could phrase that into "radiation harmful for humans / other living beings". While the radiation nuke creates is a bitch, it's nowhere the biggest problem a nuke can generate. Most of the radiation will be gone relatively soon after the explosion.
Actually, studies show that the initial radiation at Chernobyl were higher than the average airburst nuclear weapon. It's only when compared to groundburst nuclear weapons but the "fallout" (RE: lingering radiation) from Chernobyl is still greater in comparison.
Either way, check out page 8 of this document and hell read the whole thing. It's rather enlightening.
